Orthodontic Tooth Uncovering

Orthodontic Tooth Uncovering

Many of our patients that are undergoing orthodontic (braces) treatment have one or more teeth that didn’t erupt out of the gum tissue. In order to help the orthodontist, the surgeon exposes the tooth and connects a bracket with a small chain to the tooth. The patient returns to their orthodontist to start moving the tooth into its permanent location so it can be aligned with the other teeth.

What Is Orthodontic Tooth Uncovering?

Orthodontic tooth uncovering, also called surgical exposure, is a surgical therapy procedure that gives an impacted or unerupted tooth – most often a canine – a clear path to come into alignment with the rest of the teeth. Depending on how the tooth is positioned, our team uses one of two general techniques:

  • Closed Eruption Technique – The gum tissue is repositioned back over the tooth after the bracket and chain are placed, allowing it to erupt more naturally as the orthodontist guides it into place.
  • Open Eruption Technique – The tooth is left exposed to the mouth after the bracket is attached, which is typically used when the tooth sits closer to the surface.

Reasons You May Need Orthodontic Tooth Uncovering

There are a few common reasons an orthodontist will refer a patient for surgical tooth exposure, including:

  • An impacted canine or other tooth that failed to erupt on its own
  • Overcrowding that has blocked a tooth’s natural eruption path
  • An extra (supernumerary) tooth obstructing the permanent tooth
  • A baby tooth that didn’t fall out in time, blocking the permanent tooth beneath it
  • A recommendation from your orthodontist to complete your overall treatment plan

The Orthodontic Tooth Uncovering Process

Here’s what you can expect during your surgical exposure procedure:

  1. Diagnostic Imaging – 3D imaging or a digital X-ray pinpoints the exact position and angle of the impacted tooth before treatment begins.
  2. Anesthesia – Local anesthesia keeps you comfortable throughout, with sedation options available for patients who prefer extra ease during the appointment.
  3. Surgical Exposure – The gum tissue over the tooth is carefully opened, and a small bracket and chain are bonded directly to the tooth surface.
  4. Handoff to Your Orthodontist – Once healed, your orthodontist attaches the chain to your braces or aligner system and begins gently guiding the tooth into position.

After Orthodontic Tooth Uncovering – Recovery & Healing

Recovery from tooth uncovering is generally straightforward:

  • Mild swelling and tenderness at the site for a few days, manageable with over-the-counter pain medication
  • Specific aftercare instructions covering diet and oral hygiene around the treated area
  • A follow-up visit with your orthodontist, typically within one to two weeks, to begin activating the chain

Who Is a Candidate for Orthodontic Tooth Uncovering?

You’re likely a candidate for this procedure if your orthodontist has identified a tooth that hasn’t erupted on schedule and imaging confirms it’s impacted beneath the gum or bone. This is a common step for both adolescent and adult orthodontic patients whose treatment plan depends on bringing that tooth into alignment.

Frequently Asked Questions (FAQs)

What is orthodontic tooth uncovering?
It’s a surgical procedure that exposes a tooth trapped beneath the gum or bone and attaches a bracket and chain so an orthodontist can guide it into alignment.
Does tooth uncovering hurt?
No, local anesthesia keeps you comfortable during the procedure, and any soreness afterward is typically managed with over-the-counter pain medication.
How long does it take for the tooth to move into place?
Once your orthodontist begins activating the chain, movement can take anywhere from several months to over a year, depending on how far the tooth needs to travel.
Will I need this if I already have braces?
Yes, tooth uncovering is often performed mid-treatment once your orthodontist identifies a tooth that isn’t erupting on its own.
